Caltrans Bill Form

Caltrans form should only be used for Caltrans jobs. For ALL non-Caltrans jobs we recommend using the NTS Special Form and possibly the NTS Universal Form. Caltrans has two forms - Short Form and Long Form depending on number of different charges and the Short Form does not have any subtotal or total extensions or equipment rates.
